I leased/financed some equipment. Can I get a loss payee?

2. What is the full mailing address of the Loss Payee as they would like it to appear on the certificate?
3. What is the relationship between your company and the company requesting the loss payee?
4. What is the loan amount, per your loan agreement?
5. What is the account number, per your loan agreement if applicable?
6. Does the Loss Payee also require an Additional Insured certificate?
